Multiple extensions on 1 tile

I have no idea how it happened but I have 2 extensions on the same tile.
The construction sites have been manually set.

Same thing, I manually set them and I'm not sure how they were doubled up.

I have done this as well when manually placing construction sites. not sure how it happened. Room E8N2 tile 12,31.
